Several catagories are presented: Novel, Short Fiction, Dramatic Presentation, Professional Artis, Professional Magazine, and Amateur Magazine. Each catagory is blank, allowing for convention members to write in their nomination.
In the top-left corner is a stamped number "7536" in blue with a black line and "FC-4" written below it. Near the top is "p.128" written in blue. Near the top-right corner is written "April" (cursive) and "FANTASTIC" (underlined) in black ink.
